15.10 Introduction to the Male and Female Genitalia Assessment

Genitalia assessment involves inspection and palpation.

Before beginning, ask the client to empty their bladder and bowel so that fullness will not influence the findings.

Female genitalia assessments are usually performed in the lithotomy position. Male genitalia assessment is performed in the sitting and standing positions.

Always do the assessment on bare skin.

Contextualizing Inclusivity: Transgender Genitalia Assessment[1]

Assessing the genitalia of transgender patients requires sensitivity, respect, and an understanding of the specific needs and experiences of transgender individuals. Here are important considerations for health care providers when performing such assessments:

  • Address patients using their chosen name and pronouns.
  • Familiarize yourself with terms such as “transgender,” “cisgender,” “non-binary,” and specific terms the patient uses to describe their body.
  • Establish a trusting relationship by creating a welcoming and inclusive environment.
  • Explain each step of the examination process and ask for the patient’s consent before proceeding.
  • Assure the patient that their privacy and confidentiality will be maintained throughout the assessment.
  • Ensure a private and comfortable setting for the examination.
  • Allow the patient to have a support person present if they wish.
  • Use gender-affirming language and avoid assumptions about the patient’s anatomy based on their gender identity.
  • Recognize that physical examinations can trigger gender dysphoria for many transgender patients.
  • Offer emotional support and provide reassurance throughout the exam.
